Job summary

Accord is seeking a mission-driven Waiver Case Manager (WCM) to join a team serving people with disabilities. You will evaluate, plan, and support health and human service needs on an assigned caseload.

This hybrid role requires travel within the community; on-site during training, with staff working from Midway St. Paul, MN. A $1500 hiring bonus is paid over the first year, plus comprehensive benefits and PSLF eligibility.

Qualifications

• Degree required with licensure if in social work and MN licensure preferred.
• Proficiency with Microsoft, Google, and care-related platforms.
• Background checks passability and valid driver's license required.
• One year related experience if degree is not in social work.

Responsibilities

• Develop a community support plan and goals for each person on an assigned caseload. Meet with each person regularly to evaluate progress and needed changes.
• Develop person-centered service agreements, with the person, to authorize services to meet the person's assessed needs and goals. Document changes and adjustments.
• Authorize services to meet the person’s assessed needs and goals.
• Comply with MN Health Care Programs in delivery of waivered services (such as CADI, BI, EW, DD). Work with assessors to complete MnChoices assessments.

Job description

Accord is seeking a mission-driven Waiver Case Management (WCM) to join a team of Case Managers in making a difference in the lives of people living with disabilities. The WCM will evaluate, create, and support the health and human service needs of people that Accord serves.

This is a hybrid role with offices located in Midway St. Paul, MN. The WCM will be expected to travel within the community to meet with supported persons. Staff will work on-site during training and orientation.

$1500 Hiring Bonus paid over one year!

The WCM will:

• Bachelor’s Degree in Social Work, Human Services, Psychology, or related field. If degree is in social work, current MN licensure is required. Bachelor’s degree in non-Human Services field must possess one year of related experience.
• Proficiency in Microsoft, Google, and care-related platforms.
• Ability to pass mandated background checks.
• Current Driver’s license and clean driving record with reliable, insured vehicle.

Preferred qualification:

• Electronic Health Record experience.

Knowledge, Skills & Abilities:

• Strong analytical and problem-solving skills with attention to detail.
• Excellent written and verbal communication skills including the ability to adapt coaching approaches to varied learning styles.
• Customer service mindset.
• Ability to work independently and as a part of a team in sharing information and completing work, collaborating with diverse stakeholders.
• Strong documentation skills with ability to stay current on high-volume paperwork.

Compensation:

$23-25 per hour, nonexempt, depending on qualifications.
