Job summary

Reach for Resources, Inc. is seeking a CADI/BI Waiver Case Manager to provide waiver case management services to adults or minors eligible for the CADI or BI Waiver.

You will help individuals obtain and coordinate supportive services addressing basic social, recreational, health, educational, and vocational needs, managing a caseload of up to 45. Responsibilities include meeting twice yearly, coordinating funded services, maintaining confidentiality, completing paperwork, and building community

Qualifications

• Bachelor's degree in social work, psychology, sociology, or human services.
• Experience with individuals with cognitive disabilities and/or mental illness.
• Ability to pass a criminal background check and maintain transportation.

Responsibilities

• Provide case management services in accordance with DHS and county requirements.
• Meet with each individual on caseload at least twice annually.
• Coordinate waiver-funded services and assist individuals to pursue goals.
• Maintain confidentiality and complete required intake and authorization paperwork.
• Develop a person-centered plan and connect individuals with community resources.
