VNA Health Care
Overview
This position provides assessment, planning, coordination, and advocacy services for patients who may be pregnant. It involves delivering non-clinical educational information, wellness programs, and organizing all additional VNA health-related services for obstetric patients. The role also includes tracking and documenting the effectiveness and outcomes of obstetric-based services at VNA Health Center Clinic sites.
Responsibilities include:
- Assessing patients requesting verification of pregnancy status.
- Conducting pregnancy testing.
- Providing patients with pregnancy test results and reviewing VNA services to support the patient.
- Initiating benefit eligibility based on income and program requirements, including medical presumptive eligibility (MPE).
- Scheduling OB-GYN surgeries, processing maternal fetal medicine (MFM) referrals, sending prenatal records to delivering hospitals, and processing transfer requests.
- Uploading OB medical records to patient charts for the assigned location.
- Providing patient education materials.
- Following up with patients through their first prenatal appointment.
- Documenting all instructions and follow-up contacts in the patient electronic medical record (EMR).
- Informing patients of available resources and assisting in making appointments as needed.
- Evaluating the effectiveness of health education and wellness screening reminders, and coordinating additional education and care as needed.
- Maintaining all patient care coordination records and reports.
- Networking effectively and developing key contacts with community and government-based agencies.
- Ensuring phone calls and information requests are handled promptly and courteously.
- Ability to work all Health Center hours as needed.
- Working effectively and efficiently in the EMR system.
- Familiarizing and complying with all VNA policies and procedures.
- Participating in staff meetings and in-services, and demonstrating interest in personal growth and VNA’s mission/goals.
- Meeting behavioral standards and following HIPAA guidelines for protected health information.
- Traveling to other VNA health center sites for training and team meetings.
- Performing other duties as assigned.
Minimum qualifications include:
- High school diploma required; some college preferred.
- Bi-lingual in Spanish/English (reading, writing, speaking) required.
- Effective organizational and communication skills.
- Community health experience preferred.
- Experience working with pregnant women preferred.
- Excellent computer skills, especially in Microsoft Word and Excel.
