Responsibilities

• Interview mothers and families to collect required demographic data.
• Complete and submit birth certificate information within mandated timelines.
• Verify birth data against medical records for accuracy.
• Assist parents with birth certificate worksheets and answer questions.
• Process Acknowledgments of Paternity and related legal docs as needed.
• Ensure records are accurate and compliant with regulations.
• Enter data into VERI and hospital systems.
• Monitor pending records and follow up on incomplete docs.
• Maintain confidentiality of patient information.
• Prepare reports and maintain birth registration records.
• Coordinate with state vital statistics offices for corrections and reporting.
• Educate patients on obtaining certified birth certificates.
