About Us

The VNA HOSPICE team is an interdisciplinary group that works collaboratively to ensure our patients have dignity, joy, and comfort as they move toward the end of their lives. We support and educate patients, families, and caregivers throughout this journey. After death, we provide ongoing support to those left behind through our community bereavement program.

About The Job

Performs delegated nursing, homemaking, and other duties for patients of all ages and families in their home or other designated facilities. May perform PT/INR checks as ordered by the Physician and report results according to policy.

Responsibilities
  • Adhere to patient care plans as set by the primary nurse/therapist while maintaining a safe environment.
  • Demonstrate competency in patient care and treatments.
  • Reinforce patient and caregiver education provided by the primary nurse/therapist.
  • Apply problem-solving and quality improvement methodologies to patient care and organizational processes.
  • Complete documentation in clinical records, time sheets, and quality logs.
  • Attend at least 12 hours of in-service training annually.
  • Serve as a resource to students, peers, and other departments in patient care provision.
Qualifications
  • Education: High school diploma or equivalent preferred.
  • Experience: 6 months to 1 year in a related field preferred.
  • Licensure: Current Nursing Assistant License in New Hampshire required.
  • Schedule: Variable hours, including weekends, evenings, nights, and on-call, based on VNA needs. Must be willing to cover for absences and work in the assigned service area.
  • Other: Valid driver’s license and automobile insurance coverage required for travel throughout New Hampshire.
