Field visits require a valid license and reliable vehicle. The role emphasizes interdisciplinary collaboration, timely documentation, and ongoing quality improvement.

Qualifications

  • Active RN license in the state of employment.
  • CHPN and/or BSN preferred.
  • Minimum 1 year of hospice nursing experience; 2 years preferred.
  • Active Basic Life Support (BLS) certification.
  • Strong written and verbal communication skills.
  • Valid driver’s license with reliable vehicle and proof of insurance.

Responsibilities

  • Perform focused nursing assessments and address acute needs of hospice patients.
  • Ensure continuity of care through effective communication with the interdisciplinary team.
  • Complete initial and comprehensive assessments of patients and families to determine hospice needs.
  • Develop and revise individualized care plans in collaboration with the patient and caregiver.
  • Administer prescribed medications and educate families on care techniques.
