Responsibilities

• Provide virtual nursing rounds via video conferencing and assess patients.
• Develop, implement, and monitor care plans in collaboration with care teams.
• Coordinate patient discharge planning and ongoing education for patients and families.
• Lead or participate in unit governance activities and staff development.
• Document encounters and report program outcomes to support quality improvement.
