Job summary

firsthand is seeking a Centralized Health Guide in Tacoma, WA to lead the medical aspects of virtual care for individuals with serious mental illness. This role requires performing whole person assessments, collaborating with community providers, and supporting individuals in their care journey.

Your responsibilities will include providing exemplary customer service, reducing ER visits, and guiding a team of health guides. The ideal candidate will have a passion for serving individuals battling mental health challenges and 3+ years of experience as a Nurse Practitioner.

Qualifications

• 3+ years of clinical care experience, ideally in primary care or emergency department.
• Willingness to become certified in psychiatric nursing (on the RN level).
• Experience managing a population with behavioral health and/or SUD conditions.

Responsibilities

• Perform virtual whole-person clinical assessments, including history and physical examination.
• Treat conditions including prescribing medications on an ongoing and short-term basis.
• Provide urgent-care treatment for various health conditions.
