Job summary

Elevance Health is seeking a Nurse Practitioner for a 100% virtual CareBridge position in Indiana. The role requires an active RN license, MS in Nursing, and NP certification, with collaboration across physicians to develop complex care plans.

In-person trainings occur; hybrid/work-from-home arrangements apply under policy. The NP will provide telehealth care, coordinate with interdisciplinary teams, and educate patients while maintaining records and pursuing ongoing education.

Qualifications

MS in Nursing required.
Active national NP certification required.
Indiana NP license (Family or Adult) required.
RN license valid and active required.
EMR experience required.
2+ years managing complex care cases required.

Responsibilities

Provide primary and urgent health care via telephone and tele-video to home and community-based patients.
Develop and implement plans of care for adults with chronic/complex conditions.
Gather histories, perform exams, and coordinate treatment plans with the inter-disciplinary team.
Close gaps in care and ensure alignment with state regulations and policies.
Educate patients/families on medications, adherence, nutrition, and crisis prevention.
Maintain accurate medical records and participate in continuing education.
