Job summary

Accompany Health, Inc. seeks a Med-Psych Advanced Practice Clinician (APC) to deliver high-quality care across medical and behavioral health in a BH-first model. The role emphasizes managing complex, comorbid patients in virtual settings with a longitudinal approach.

The APC will work within a multidisciplinary team, provide care virtually and at home, coordinate with health advocates, and focus on evidence-based practice to keep patients healthy in their homes.

Qualifications

• Active NP or PA license in the state of employment.
• DEA registration or eligibility to obtain.
• Experience with DSM-5 criteria and high-risk populations.

Responsibilities

• Provide longitudinal biopsychosocial care for complex patients.
• Manage chronic psychiatric and medical conditions with BH team.
• Prescribe OUD/OUD meds per supervision and manage SUD therapy.
