The APC will serve patients remotely across the United States, collaborating with a multidisciplinary team to provide longitudinal care and evidence-based treatment. The role emphasizes autonomy, innovation, and in-home BH support, with a strong focus on keeping patients healthy at home and reducing hospital use.

Qualifications

• APC with clinical expertise in primary care and psychiatric management.
• Experience with serious mental illness or substance use disorders in a BH-first model.
• Licensed to practice as an APC in the United States.

Responsibilities

• Conduct virtual onboarding and follow-up visits with a Health Advocate.
• Diagnose and manage chronic medical and behavioral health conditions.
• Provide preventive care and close care gaps.
• Prescribe treatments for OUD and AUD per BH-first model.
