Qualificações

• Board Certified as a Family Nurse Practitioner.
• Active Family Nurse Practitioner license in Louisiana.
• Consistently exhibits professionalism, integrity, accountability, confidentiality, care and compassion.

Responsabilidades

• Review case load and prepare technology at start of day.
• Navigate schedules with Sprinters to visit patients at home.
• Create meaningful connections with patients and collect relevant information.
• Conduct wellness visits, health risk assessments, and patient education.
• Collaborate with Sprinters to evaluate vital signs and arrange blood draws.
• Provide comprehensive care coordination and documentation for patients.
