Successful candidates are Board Certified Family Nurse Practitioners with an active Louisiana license, comfortable with telehealth tools, and capable of working independently

Qualifications

• Board Certified as a Family Nurse Practitioner.
• Active Family Nurse Practitioner license in Louisiana.
• Consistently exhibits the highest levels of professionalism, integrity, confidentiality, care and compassion to provide high quality health services.
• Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking.
• Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
• Strong written and verbal communication skills
• Ability to work independently or in a team environment
• Pass national background check and valid clinical license search

Responsabilités

• Review your case load and prepare your technology, ensuring you have all tools to service patients.
• Navigate through your schedule and partner with Sprinters that will visit each patient’s home.
• Create meaningful bonds and connections with diverse patients as you collect information and perform services.
• Engage in proactive care with health risk assessments, medication reviews, and cognitive screenings.
• Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs.
• Provide comprehensive care coordination and documentation of findings.
