Included Health is seeking board-certified Family Nurse Practitioners to deliver high-quality virtual primary care to patients nationwide via video and phone visits. You will manage your own patient panel, collaborate with a multidisciplinary team, and maintain excellent patient relationships while adhering to HIPAA and state guidelines.

Qualifications

• Active, unrestricted NP license in state of residence.
• Board certification by ANCC or AANP is required.
• Experience delivering primary care in ambulatory/outpatient settings is required.
• Ability to obtain additional state licenses as needed.

Responsibilities

• Provide high-quality virtual primary care through video visits, including assessment, diagnosis, treatment, and patient education.
• Manage a panel of patients, delivering comprehensive care across acute, chronic, and preventive needs.
• Collaborate with the Care and Case Management team to develop care plans and support successful outcomes.
• Partner with supervising physicians as required by state and practice guidelines.
• Work with the Credentialing Team to obtain additional state licenses as needed.
• Maintain patient satisfaction and quality metrics by practicing evidence-based medicine with exemplary bedside manner.
• Utilize telehealth technology to deliver patient care, while effectively managing multiple tasks in a fast-paced clinical environment.
• Stay current with medical knowledge, treatments, and medications.
• Adhere to HIPAA and security regulations to safeguard patient information
• Contribute to a culture of humility, curiosity, and collaboration.
