CommonSpirit Health's St. Luke’s Health – Memorial Lufkin seeks a Virtually Integrated Care Medical Surgical Nurse (RN) to lead virtual care teams, coordinate patient plans, and deliver evidence-based nursing in a dynamic, technology-driven environment.

The role requires Texas RN licensure, BSN or active pursuit, 3 years Med Surg in acute care, preceptorship, 2 years in leadership, and BLS; CAVRN within 12 months. Opportunity exists to transition to remote work after orientation.

Qualifications

• Must be licensed as RN in Texas.
• Minimum 3 years RN Med Surg in an acute care facility.
• 3 years nursing preceptor experience.
• 2 years in a nursing leadership role (Charge RN eligible).
• BLS certification through AHA.
• CAVRN certification to be obtained within 12 months.

Job Summary and Responsibilities

The Virtually Integrated Care (VIC) Medical Surgical Nurse works as the catalyst for patient centered care by leading a well-coordinated care team. The VIC RN is at the center of innovation and a transformational, evidence based practice care model. The experienced RN in this role will anticipate and direct the care of the patient through evaluating health promotion, risk reduction and clinical decision-making. The Nurse assumes accountability for client care outcomes through the assimilation and application of evidence based practice to design, implement, and evaluate client plans of care in a virtual environment setting. This role functions as the center of coordination and communication for a multidisciplinary team. A high degree of professional competence and autonomy will be demonstrated in performing the roles of 1) clinician, 2) educator, 3) consultant/collaborator, 4) leader. The VIC RN will function as a dynamic change leader for this transformational care model. The role is facility based for completion of orientation and has the potential to transition to a remote role with successful implementation of the care model.

Job Requirements

• Bachelors in Nursing, BSN degree OR Bachelors Of Science (Actively in pursuit of Bachelors in Nursing, BSN degree)
• Licensed to practice as a Registered Nurse in the State of Texas
• Three (3) years of RN Med Surg experience in an acute care facility
• Three (3) years of nursing preceptor experience
• Two (2) years in a nursing leadership role (Charge RN role applicable)
• Basic Life Support (BLS) through American Heart Association (AHA)
• Certified Acute-Care Virtual Registered Nurse, CAVRN (within 12 months, or 1000 hours worked)

Preferred

• Five Plus (5+) years of RN Med Surg experience in an acute care facility
• Masters in Nursing, MSN
