Job summary

Jobtailor in Austin, TX is seeking a skilled home health professional to coordinate patient assessments, enable video telehealth reviews, and manage complex discharge planning to support safe, effective care at home.

You will escalate deteriorating patients, provide on-call support, and develop individual care plans that enable virtual care where possible while partnering with patients, families, and treating teams.

Job description

Responsibilities

• Coordinating patient assessments to provide home nursing services, facilitating video telehealth reviews, and completing complex discharge planning.
• Supporting escalation of the deteriorating patient - both through supporting visiting nurses remotely, and through the provision of on-call support and after-hours escalation.
• Formulating individual plans for patients to support their complex needs, aiming to provide care virtually where possible and supporting the patient and the treating team with a high level of confidence and expertise.

Requirements

• A strong level of clinical experience and confidence, allowing you to make excellent decisions on clinical escalation and care provision.
• A collaborative mindset, working with the treating teams, care partners, the patient and their carer to provide high quality, evidence-based care at home.
• A flexible, person centred approach to care, implementing care plans for patient to allow them to get home as soon as possible.
• A current, unrestricted Driver's license.
