Job summary

Rincon Family Services in the Chicago area seeks a CC-JAC Case Manager to provide case management for individuals and families affected by community violence. You will conduct intake, develop service plans, connect to resources, advocate, and document progress with a team of staff and partners.

The role requires trauma-informed, culturally competent communication and the ability to work with diverse communities.

Qualifications

• Bachelor’s degree in social work or related field.
• 5–10 years in case management or related social services.
• Knowledge of Chicago-area resources.
• Bilingual Spanish/English preferred.

Responsibilities

• Conduct intake and assess needs to develop individualized service plans.
• Provide ongoing case management, follow-up, advocacy, and support to participants and families.
• Connect participants to community resources and partner agencies.
• Follow up on referrals and monitor progress and outcomes.
• Maintain case notes, records, and required program data.
• Use electronic case management and reporting systems (Salesforce).
