Job summary

The Sonoma Post Acute is seeking an Activity Assistant to support the Activity Director in delivering a creative, engaging activities program that meets residents' physical, mental, and psychosocial needs in line with applicable regulations and facility guidelines.

You will plan and conduct individual and group activities, develop monthly calendars, arrange transportation when needed, assist with quarterly progress notes, and help communicate with residents, families, staff, and community

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read and comprehend policy and procedure manuals.
• Ability to read technical procedures.

Responsibilities

• Participate in planning and conducting activities for residents.
• Assist in communication between staff, residents, families, and the public.
• Develop monthly activity calendar and maintain attendance records.
