Job summary

VA Northeast Ohio Healthcare System seeks a Registered Nurse Telehealth Case Manager within Community Outpatient Services. You will provide evidence-based nursing care, coordinate patient management, and plan discharge/disposition for inpatients and outpatients.

You will collaborate with Veterans, families, and caregivers to arrange the most appropriate options and services for complex health needs, including telehealth encounters and care transitions.

Qualifications

• Graduate of a school of professional nursing approved by the appropriate accrediting agency and licensed as a registered nurse
• Experience coordinating patient care across inpatient and outpatient settings is preferred
• Ability to work within VA policies and telehealth platforms

Responsibilities

• Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings
• Assists Veterans, family members, and caregivers with options and services to meet complex health care needs
• Delivers telehealth clinical encounters and coordinates care across PACT and specialty services
