Responsibilities

• Assist in administrative and clinical operations of Social Work Services.
• Serve as the initial point of contact for veterans and determine their initial needs for VA services.
• Coordinate outreach events with the military and other providers; provide logistical support for the program.
• Maintain knowledge of community supports and resources for veterans; identify gaps in services.
• Ensure veteran enrollment in VA health and schedule intake assessments.
• Complete initial assessments to determine immediate and long-term needs; plan to meet identified needs.
• Identify case management needs and coordinate with Social Work and Nurse Case Managers.
• Receive external and internal consults for TCM case management.
• Engage in staff and provider education; present briefs on benefits and VA health care at events.
