Job summary

Primavera Foundation in Tucson, AZ seeks a Rural Resource Specialist to help veterans and their families transition to permanent housing. You will conduct intake, develop housing plans, and coordinate services with partners.

The role emphasizes advocacy, data entry in HMIS, confidentiality, and reporting. You will support rapid rehousing and homelessness prevention while adhering to program guidelines and Primavera’s principles.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Minimum of three years in social services providing case management.
• One-year experience with low income, homeless, or veterans.
• Note: combination of education and experience may be considered.
• First Aid/CPR certification, or ability to obtain.
• Level 1 fingerprint clearance card, or ability to obtain.
• Background checks.
• Reliable transportation, valid driver’s license, clean driving record, and proof of insurance.

Responsibilities

• Conduct intake/eligibility meetings and determine eligibility.
• Develop housing service plans and enter data in program database.
• Assist participants in identifying, applying for, and moving into permanent housing.
• Coordinate with leadership and communicate approvals and follow-up with participants.
• Follow up with participants and conduct 90-day re-certifications per guidelines.
• Communicate with other service providers on a regular basis.
• Assist with applications for mainstream benefits and advocate for participants.
• Maintain confidentiality of participant information.
• Complete monthly reports on caseload status.
• Participate in VA and HMIS trainings and other agency trainings.
