Job summary

You will assess, diagnose, treat, and educate patients across outpatient services, with emphasis on preventive care and chronic disease management. The position supports collaboration within a Patient Aligned Care Team, employs evidence-based practices, and may involve telehealth modalities.

Qualifications

• The NP is a licensed independent practitioner with advanced education and national certification.
• Engages in evidence-based care for a Veterans patient panel.
• Collaborates with the Patient Aligned Care Team to provide timely, quality care.

Responsibilities

• Provide comprehensive primary care to male and female Veterans.
• Conduct physical exams and develop treatment plans.
• Order and interpret labs and diagnostic studies; prescribe pharmacologic and non-pharmacologic therapies.
• Coordinate referrals to specialty services and ensure health maintenance.
