Job summary

Diversity Group is seeking a Nurse Practitioner to provide comprehensive primary care for Veterans within the Durham VA Health Care System. The NP will perform independent clinical evaluations, develop care plans, and coordinate with a Patient Aligned Care Team.

The role emphasizes evidence-based practice, prevention, and patient education, with potential telehealth components and a collaborative practice agreement for prescribing controlled substances. EDRP eligibility applies where appropriate.

Qualifications

• Licensed NP with advanced education and national certification.
• Collaborative practice agreement required for prescribing controlled substances.

Responsibilities

• Independent clinical evaluation of Veterans through panel of patients.
• Provide comprehensive primary care including health promotion and disease prevention.
• Develop and implement individualized plans to achieve population health goals.
• Order and interpret labs and diagnostics, prescribe pharmacologic and non-pharmacologic interventions.
• Collaborate with Patient Aligned Care Team to ensure timely, quality care.
