Veterans Health Administration (DOVA) is seeking a Social Worker for the outpatient PACT clinic in San Diego, CA. The role supports Veterans in need of social services and collaborates with the multidisciplinary team to ensure holistic care.

The position requires a Master’s degree in Social Work, state licensure for independent practice, and U.S. citizenship. The schedule is part-time (20 hours) with non-remote work in San Diego.

Formación

• Master’s degree in social work from CSWE-accredited program.
• Current state license to independently practice social work.

Responsabilidades

• Provides outpatient social work support in the VA PACT clinic.
• Collaborates with the multidisciplinary team to deliver services.
• Assesses Veterans for psychosocial needs and eligibility for VA programs.
• Recommends community resources and care planning.
• Educates Veterans on Advance Directives and care options.
• Supports homeless resources, food resources, and housing placements.
