Job summary

The Primavera Foundation is seeking an Outreach Specialist to connect veterans and families to housing and supportive services in Tucson as part of the PAV (Project Action for Veterans) program. You’ll coordinate outreach, intake, and referrals, working with the VA and community partners to prevent homelessness and secure resources.

Ideal candidates have a degree in social services, outreach experience, strong communication, and a commitment to integrity and teamwork.

Qualifications

Bachelor’s degree in social service or related field.
Minimum of one year of case management or street outreach experience with homeless or low-income families.
Proficiency with Microsoft Office and web-based data systems.
Strong verbal, written, group presentation, and time management skills.
First Aid/CPR certification or ability to obtain.
Level 1 fingerprint clearance card or ability to obtain.
Successfully complete the background check process.
Reliable transportation with valid license, registration, and proof of insurance.

Responsibilities

Identify and interview participants, assess needs during street outreach and coordinate intake.
Coordinate intake appointments with Resource Specialists and team members.
Cover walk-in hours in the absence of the Program Assistant and when backup help is needed.
Network with community members and the VA to market the program.
Inform VA staff of changes and encourage referrals.
Provide information, referrals, and schedule follow-up assistance.
Work with Primavera staff and partners on participant needs.
Serve as backup for apartment habitability inspections as requested.
Develop and maintain an outreach schedule with agencies and VA personnel.
Maintain an outreach log and report weekly to the Co-Director.
Adhere to SSVF policies and updates.
Maintain confidential assessment files and data accuracy.
Assist Resource Specialists to meet grant/program requirements.
Attend outreach events and meetings for SSVF and PAV team.
Report emergencies to leadership; attend All Staff meetings.
Perform other duties as assigned; uphold integrity, respect, accountability, compassion, leadership.
