Job summary

U.S. Department of Veterans Affairs is seeking a Clinical Social Worker for the Home Based Primary Care (HBPC) Program. The role provides direct services, case management, advocacy, and care coordination with VA and community resources within a multidisciplinary team.

Required are an MSW from a CSWE-accredited program, state licensure, and a valid Driver's License; English proficiency is essential. Opportunities exist for GS-9 to GS-11 grade levels depending on experience and credentials.

Qualifications

• Master's degree in social work from a CSWE-accredited school.
• Licensed or certified by a state to independently practice social work.
• Driver's License required.
• Citizenship and English proficiency required.

Responsibilities

• Provide direct services to Veterans in HBPC with case management, clinical treatment, advocacy, and linkage to VA/community resources.
• Collaborate with HBPC team or interdisciplinary treatment teams to coordinate care.
• Assess psychosocial functioning and develop/implement treatment plans.
• Utilize various treatment modalities with individuals, families, and groups to reach goals.
