Qualifications

• 2 years of experience in nonprofit or social services.
• Understanding of issues related to homelessness and community resources.
• Experience with individuals experiencing homelessness and with mental health/substance use conditions.

Responsibilities

• Assist veterans in accessing VA and mainstream healthcare systems.
• Build and maintain relationships with healthcare providers.
• Participate in community-based outreach activities.
• Ensure documentation and reporting of required information.
• Work as part of the VHS team to achieve program goals.
