Texas Panhandle Centers (TPC) is recruiting a Veterans Case Manager (1456) to deliver trauma-informed care, skills training, and treatment planning for adults in Amarillo and the Texas Panhandle region. This full-time role supports veterans and families within a multidisciplinary team.

Qualifications

• Bachelor’s degree from an accredited university in Psychology, Sociology, Social Work, Counseling, or a related Human Services field required for QMHP credentialing.
• Current or previous enlistment in the U.S. armed forces.
• Valid Texas Driver’s License with an insurable driving record.
• At least 18 years of age.
• Employees may be required to utilize personal mobile phones for 2FA.

Responsibilities

• Maintain ethical and professional communications using trauma-informed language.
• Participate in the SSG Fox grant initiatives to address mental health and social determinants of health.
• Provide outreach to identify eligible Veterans at risk and educate families and communities on suicide prevention.
• Conduct crisis responses and coordinate with individuals at risk of crisis.
• Develop and implement crisis intervention plans tailored to each Veteran.
• Build rapid rapport and empower Veterans in decision-making and goal setting.
• Apply trauma-informed care principles to create a safe environment.
• Assist Veterans in obtaining VA or other benefits they qualify for.
• Assist with data evaluation to comply with grant requirements.
• Train providers to offer groups, care coordination, and crisis-related practices.
• Participate in team meetings and case staffing for coordinated care.
• Conduct follow-ups in the community and provide referrals to VA or local resources.
• Maintain documentation and compliance for all individuals served.
• Perform other essential functions as needed and adhere to the code of conduct.
