Job summary

Primary Care Solutions, Inc. is seeking a skilled Nurse Practitioner to join our VA Community-Based Outpatient Clinic (CBOC) team. You will deliver high-quality care to US Veterans, collaborating with the PACT team to develop and implement patient management plans and ensure adherence to clinical standards.

The role emphasizes leadership, patient-centered care, and adherence to VA IT systems (VISTA/CPRS). A strong nursing background and board certification in family practice or internal medicine

Qualifications

• Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).
• Board-certified in family practice or internal medicine.
• Certification by an applicable professional organization.
• 1 year of clinical medical practice preferred.
• Current, unrestricted license in the state of desired employment.
• Current BLS for providers from the American Heart Association (to be renewed annually).
• Current, unrestricted DEA registration.
• Excellent computer skills.

Responsibilities

• Examine the patient/client and determine the best course of action while documenting in our paperless health program – The VA’s VISTA/CPRS electronic patient record system.
• Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.
• Discuss patient outcomes, policies, procedures, and records with Medical Director.
• Completes all clinical reminders to keep each patient up to date in the system.
