Primary Care Solutions, Inc. seeks a Nurse Practitioner to join our VA Community-Based Outpatient Clinic (CBOC). You will provide patient-focused care to US Veterans and collaborate with the Patient Aligned Care Team to ensure high-quality outcomes.

The position offers a Monday–Friday schedule, no on-call, competitive pay, and comprehensive health, retirement, and time-off benefits. A state license, MSN, board certification in family or internal medicine, and VA clinical experience preferred.

Qualifications

• Graduation from an accredited MSN program.
• Board-certified in family practice or internal medicine.
• Certification by an applicable professional organization.
• 1 year of clinical medical practice preferred.
• Current, unrestricted license in the state of employment.
• BLS for providers (AHA) current.
• Current, unrestricted DEA registration.
• Excellent computer skills.

Responsibilities

• Examine patients and determine the best course of action and document in VA VISTA/CPRS.
• Develop and implement patient management plans with the PACT team.
• Discuss patient outcomes with Medical Director and colleagues.
• Complete all clinical reminders to keep patients up to date.
