Qualifications

• KSAs: ability to provide a range of patient/resident care and contribute to the nurse care plan.
• Observation of patient status and timely reporting of deviations.
• Ability to communicate effectively with patients, families, and health professionals.
• Ability to recognize emergent care situations and intervene as needed.

Responsibilities

• Maintain therapeutic relationships and observe patient conditions.
• Document care, observations, and responses in appropriate records.
• Assist with activities of daily living and basic nursing procedures under RN/LPN supervision.
• Participate in treatment planning meetings and shift reports.
• Ensure a safe, clean environment and proper infection control practices.
• Serve as a preceptor to other NAs and orient new staff.
