Job summary

Friendship Place is seeking a Case Manager for the Maryland Office to deliver shallow subsidy, homeless prevention, and rapid re-housing services to low-income Veteran households. The role focuses on implementing Shallow Subsidy and developing housing stability plans within a Housing First framework.

You will work primarily in the field with participants, with some office time for admin and supervision. The position is hybrid, with hours 7:00 AM–8:00 PM and occasional evenings, holidays, and

Qualifications

• A bachelor’s degree is preferred.
• 1 year of experience in a similar setting is preferred.
• Must have or be able to obtain First Aid/CPR/AED certification (Friendship Place will provide training).
• A valid driver’s license, current auto insurance, and ability to operate a motor vehicle are required.
• Understanding and experience working with Veterans is preferred.
• Proficient in Microsoft Programs (Word, Excel, PowerPoint, Outlook) internet services and web-based reporting systems are required.

Responsibilities

• Provide shallow subsidy case management services to at least 50 low-income Veteran participants consistently throughout a 2-year period.
• Develop, implement, and revise 90-day and 2-year housing stability plans with Veteran participants.
• Monitor income changes and assist with budgeting at least quarterly.
• Deliver face-to-face contact to participants.
• Foster wellness and independence.
• Engage participants and assess individual needs, preferences, and strengths.
• Assist participants in locating or maintaining permanent housing.
• Make appropriate referrals to identified needed services.
• Act as a liaison with landlords and other community stakeholders.
• Collaborate with agencies providing services to participants, including community, government, and VA.
• Participate in multidisciplinary meetings and regular supervision.
• Maintain paper files
• Maintain participants' electronic records in the Homeless Management Information System.
• Demonstrate empathy and flexibility in responding to participants' needs and requests.
• Complete SOAR training and become certified to submit SOAR applications to increase a household's income
• Attend professional trainings and conferences as needed.
• Perform all other duties as assigned
