Qualifications

• RN license in the United States is required.

• Perform case management, assessment, triage, and Veteran education.
• Coordinate resources, medications, treatments, and care planning for Veterans with complex needs.
• Deliver nursing care in the home and promote health and prevention for geriatric and palliative needs.
