Job summary

The Mankato Community Based Outpatient Clinic (CBOC) is recruiting a Full-Time Staff Physician to join the VA health care system in Minnesota. The role emphasizes primary care in a collaborative, team-based setting with access to telemedicine and on-site lab and x-ray capabilities.

The provider will independently manage a patient panel with acute and chronic conditions, develop care plans for population health, and coordinate referrals to specialty services.

• Emphasis on health promotion and disease prevention using evidence-based practice.
• Ability to develop and implement individualized plans for population health and disease management.
• Leadership and collaboration with primary care team and interdisciplinary members.

Responsibilities

• Independently manages a panel of patients with acute and chronic health conditions.
• Develops and implements an individualized plan to achieve optimal population health and disease management.
• Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals and consults to specialty services.
• Provides education to patients and their families.
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
