The provider will independently manage a patient panel with acute and chronic conditions, develop care plans for population health, and coordinate referrals to specialty services.

The Mankato Community Based Outpatient Clinic (CBOC) has an opening for a Full-Time Staff Physician. CBOCs within the Minneapolis VA Health Care System provide care to Veterans near their home town and serve over 50 -000 Veterans throughout Minnesota and Western Wisconsin. Duties

• Seeking a APRN trained in primary care provider is sought for a VA clinic in Mankato - Minnesota -part of VA health care system
• Our facilities include an updated fully-equipped medical clinic with a lab - x- ray and telemedicine capabilities
• Our physicians/Providers serve the primary care health needs of Hibbing's and rural areas using a team approach with nursing and onsite clinical pharmacist
• Mental Health services within the clinic help to provide comprehensive care to the Veterans served in this VA Community Clinic.Duties include: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice

• Provider will be required to be a designated women's provider and complete the mandatory VHA training
• 13 month NTE position will be discussed during the interview process
