Job summary

The U.S. Department of Veterans Affairs' VA Ann Arbor Healthcare System seeks a Social Worker to provide outpatient services at the Jackson CBOC, serving Veterans in PACT, specialty clinics, and triage walk-in.

You will perform psychosocial assessments, develop treatment plans, coordinate care, and collaborate with interdisciplinary teams. Licensure and CSWE-accredited MSW required; GS-9 to GS-11 eligible; Monday–Friday schedule; relocation incentives may apply.

Qualifications

• Master's degree in social work from a CSWE-accredited program
• Licensure/certification to independently practice social work in state (as applicable)
• GS-9 level requires one year of post-MSW experience or doctoral substitution allowed by VA standards
• Knowledge of community resources and referrals

Responsibilities

• Provide outpatient social work services in PACT, GM & Specialty clinics and triage walk-in clinic
• Conduct psychosocial assessments and formulate treatment plans
• Coordinate care with Veterans, families, and interdisciplinary teams
• Deliver patient-centered interventions to address emotional, social, physical, and economic needs
• Participate in crisis intervention and advocacy efforts
• Manage referrals to community and government resources
