U.S. Renal Care is seeking a VBC Care Coordinator to partner with the Value-Based Care Clinical team. This remote position is based in Texas and supports scheduling, patient questionnaires, and care-management workflow across our patient population.
You will collect and share vital documentation, ensure timely data entry in the care platform, and promote high-quality patient interactions. Experience in healthcare or call centers is preferred, and bilingual Spanish/English is required.
The VBC Care Coordinator partners with our Value-Based Care Clinical team to drive improved outcomes for chronic kidney disease and end-stage kidney disease patients. Acts as the first point of patient contact for incoming or outbound calls. Supports gathering and sharing of key documentation (e.g., discharge summaries) to drive care. Manages workflow in our care management platform.
Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.
Make outbound or receive inbound calls from patients to schedule appointments and/or address program needs.
Conduct health-related patient questionnaires, as needed.
Documents all activities for quality and metrics reporting through the care management platform.
Gathers and stores critical patient information to drive nurse and other care team member workflow.
May coordinate patient transportation.
Participates in continuous quality improvement initiatives to ensure department and company goals are met and exceeded.
Maintains performance and quality standards based on established contact center metrics.
Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.
Participates in team concepts and promotes a team effort; performs duties in accordance with company policies and procedures.
Regular and reliable attendance is required for the job.
Remote position, prefer candidates based in Dallas and San Antonio.
