• Collects and analyzes data for trends; enters data into registries for benchmarking and participates in performance improvement activities.
• Coordinates post-hospitalization care and discharge planning, including referrals and arranging appointments with clinics and home care providers.
• Promotes professional development and models nursing best practices through the nursing process and professional behaviors.
