UNC Health Care is seeking a clinical administrative nursing care coordination professional for UNC Medical Center, Eastowne Nursing Support Services in Chapel Hill, NC. This full-time, on-site role supports a patient care area with discharge planning, data analysis, performance metrics, and patient/staff education.
The position requires graduation from a state-accredited school of professional nursing, NC licensure as a Registered Nurse, and at least five years of professional nursing

Provides clinical administrative nursing care coordination in support of a patient care area. Duties include one or more of the following care coordination, discharge planning, data analysis and performance metrics, and patient/staff education.
$10,000 Commitment Incentive paid out in six (6) installments over three (3) years.
