Job summary

Henry Ford Health in Detroit is seeking a Nurse Coordinator (RN) to develop, implement, monitor, and evaluate a care program for patients. The role coordinates care across the continuum to ensure quality outcomes and collaborates with physicians, nurses, extenders, and support staff to assure optimal patient care and satisfaction.

The successful candidate will provide patient and family education on disease processes and will train and lead a nursing team for optimal patient outcomes.

Qualifications

• Graduate from an accredited school of nursing.
• Minimum of 3–5 years of progressively responsible clinical nursing practice in vascular specialty.
• Bachelor’s Degree in Nursing (preferred).
• Certification in vascular specialty (preferred).
• Current clinical knowledge and analytical abilities to formulate effective nursing care plans.
• Ability to perform a wide variety of professional nursing services.

Responsibilities

• Develops, implements, monitors and evaluates a program of care for patients under the Nursing Leader/Administration.
• Coordinates care across the continuum to facilitate quality outcomes.
• Trains and directs a nursing team in the optimal care of the patient.
