Job summary

Qualifications

Responsibilities

• Deliver meals from the contracted provider to designated meal sites and home-bound consumers.
• Monitor meal recipients for changing conditions and alert program staff to risks.
• Monitor, report, and document temperature of food on a regular basis.
• Distribute and collect quality assurance surveys from consumers.
• Collect and deposit consumer contributions as appropriate.
• Plan routes to maximize efficiency and timeliness.
• Deliver supplies to meal providers and nutrition program staff as needed.
• Maintain accurate daily records of meals delivered.
• Arrange regular vehicle maintenance and keep the vehicle clean.

Job description

• A. Pick up and deliver meals from the contracted provider to designated meal sites and home-bound consumers.
• B. Monitor meal recipients for changing conditions and alert program staff to conditions that may indicate a decrease or increase in functioning or an increase in risk, including but not limited to dementia, elder abuse/neglect, depression and alcoholism.
• C. Monitor quality of meals delivered and report concerns to program staff.
• D. Monitor, record, and document temperature of food on a regular and ongoing basis.
• E. Distribute and collect quality assurance surveys from consumers.
• F. Collect and deposit consumer contributions as appropriate.
• G. Plan delivery routes to be as efficient as reasonably possible.
• H. Distribute information to consumers as appropriate.
• I. Deliver supplies to meal providers and other nutrition program staff as needed.
• J. Maintain accurate daily records of meals delivered.
• K. Arrange/schedule regular maintenance of the Department’s vehicle and keep the vehicle clean and free of debris.
• L. Inform program staff of any problems with the vehicle.
• M. Report, as required by county policies and procedures, all vehicle accidents.
• N. Observe all county vehicle regulations and safety requirements.
• O. Complete required incident reports.
• P. Participate in job related training as required.
• Q. Other duties as assigned.

• A. High School Diploma or an equivalency (GED, HSED)
• B. Good Driving Record.
• C. Valid Wisconsin driver's license and reliable transportation.

Physical Requirements:

• A. Ability to sit or stand for extended periods of time while performing duties.
