Job summary

Diversicare is seeking a compassionate Certified Nurse Assistant (C.N.A.) to join our Oak Ridge team. You will assist residents with daily living, mobility, and transportation to medical appointments, prioritizing safety and dignity.

Qualifications

• > Physically able to transfer ambulatory and non-ambulatory residents between wheelchair and vehicle.
• > Possess a valid state driver’s license.
• > Certified Nurse Assistant (C.N.A.) certification.
• > Clean driving record.
• > Clean criminal record.
• > Any other state required certifications.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit: sign in resident; present documents to office personnel.
• Obtain resident’s documents and any new prescriptions; ensure doctor’s order is updated; arrange follow-up appointment if needed.
• Return to facility and report to RCM; be prepared to answer questions about the visit.
• Performs other duties as required and assigned.

• Before leaving doctor’s office, please observe the following: a. Obtain resident’s documents. b. Obtain any new prescriptions. c. Check to make sure doctor has written on the doctor’s order form. d. If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.
