Job summary

Diversicare is hiring a Certified Nurse Assistant to support residents in our Amory, MS centers. You will assist with safe transfers, accompany residents to medical appointments, and help ensure documentation and orders are properly handled. The role emphasizes care, safety, and adherence to service standards in a post-acute setting.

Qualifications

• Must be a Certified Nurse Assistant (C.N.A.).
• Must possess a valid state driver’s license.
• Physically able to assist ambulatory and non-ambulatory residents with transfers.

Responsibilities

• Ensure resident safety during transport to/from medical visits.
• Accompany resident on medical visits and stay with them until visit ends.
• Sign in resident and present documents to office staff.
• Obtain patient documents and prescriptions as needed.
• Verify doctor orders and secure appointment cards if follow-up is required.
• Report visit details to the facility RCM and be prepared for questions.
• Perform other duties as required by the role.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit:
  • Sign in resident upon entrance.
  • Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following:
  • Obtain resident’s documents.
  • Obtain any new prescriptions.
  • Check to make sure doctor has written on the doctor’s order form.
  • If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.

Qualifications

• Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
• Possess a valid state driver’s license.
• Qualified as a Certified Nurse Assistant (C.N.A.).
• Clean driving record.
• Clean criminal record.
• Any other state required certifications
