Responsibilities
  1. Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
  2. Accompany each resident on all medical visits. Stay with resident(s) until the visit is complete.
  3. Observe the following guidelines at each visit:
    • Sign in resident upon entrance.
    • Present resident’s documents to office personnel.
  4. Before leaving the doctor’s office, please observe the following:
    • Obtain resident’s documents.
    • Obtain any new prescriptions.
    • Check to make sure the doctor has written on the doctor’s order form.
    • If a follow-up appointment is necessary, obtain appointment cards.
  5. After returning to the facility, report to RCM and be prepared to answer questions concerning the visit.
  6. Perform other duties as required and assigned.
Qualifications
  • Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
  • Possess a valid state driver’s license.
  • Qualified as a Certified Nurse Assistant (C.N.A.).
  • Clean driving record.
  • Clean criminal record.
  • Any other state-required certifications.
