Job summary

Nexus Health Systems in Conroe, Texas, is seeking a dedicated transportation coordinator to oversee the safe transport of residents to their medical appointments. This role includes ensuring that all vehicles are maintained in good condition, performing preventive maintenance, and maintaining a clean environment.

The ideal candidate should have a high school diploma and preferably six months of experience in a personal care setting. A valid Texas Driver's License and effective communication skills are essential. Certification in PMAB and AHA BLS is also required for this position.

Job description

Position Summary

Responsible for coordinating the transportation needs of residents and transporting them safely to and from their appointments promptly, checking vans daily for cleanliness, and performing, upkeep and basic preventive maintenance on vans.

Job-specific Responsibilities
  • Demonstrates knowledge of individual residents' histories, current progress, and personal issues or difficulties.
  • Ensures all vehicles' exterior and interior are clean at all times.
  • Maintains an updated transportation schedule to include scheduling of vans.
  • Responsible for obtaining driving directions for all transports.
  • Provides direct van transportation services by safely driving, monitoring, and reviewing by the supervisor based on a schedule.
  • Ensures all safety equipment (i.e., fire extinguisher, first aid kits, etc.) is in each van, properly secured, and in good working condition.
  • Ensures each vehicle is updated with a proper license and current inspection sticker.
  • Ensures each vehicle has all necessary paperwork, including resident emergency information and current insurance cards.
  • Ensures repairs and all maintenance work necessary is performed on vehicles.
  • Ensures residents are transported to and from their appointments safely and promptly.
  • Ensures physician staff completes all paperwork for residents at appointment.
  • Demonstrates ability to accomplish assigned tasks with minimal supervision.
  • Displays willingness to work additional hours when necessary.
  • Demonstrates ability to interact effectively with residents, including the ability to de-escalate residents with verbal interventions.
  • Assists residents in promptly following through with medical and community appointments.
  • Communicates in a courteous, respectful, and kind manner to staff, residents, visitors, telephone callers, etc.
  • Ensures receipts are turned into the supervisor for petty cash for reconciliation.
  • Ensures all medications are administered during transportations and notifies nurses and supervisors of any missed medications, medication errors and/or medication problems.
  • Performs other duties as assigned.
Education
  • Minimum of high school diploma or equivalent
Experience
  • Preferably a minimum of six (6) months experience in transporting residents in a personal care setting.
  • Preferably knowledge of or exposure to assisting persons with neurological deficits or persons with disabilities in rehabilitation, acute care, community re-entry, or other healthcare-related programs.
Licensure/Certification
  • Must be certified in PMAB and AHA BLS.
  • Must have a valid Texas Driver's License and a good driving record.
  • Must be knowledgeable of and practice Infection Control and Universal Precautions at all times.
  • Must obtain a tuberculosis test with the first 14 days of employment and annually after that.
  • Must pass a criminal background, misconduct registry, and OIG/GSA Sanction check.
