Job summary

Western Slope Health Center is seeking a patient transport assistant to help move residents to and from appointments and activities. You will load and unload residents, operate vehicles safely, and maintain clear communication with residents and staff.

You must be able to lift up to 75 pounds, be familiar with GPS navigation, and pass a background check. This on-site role requires a reliable driver with patient-centered care experience.

Qualifications

• Valid driver’s license required.
• Familiarity with GPS and local driving area.
• No recent traffic-related felonies or DUI.
• Pass a drug screen and background check.

Responsibilities

• Assist residents with loading/unloading from facility-approved vehicles.

Job description

Overview

Assists residents with loading and unloading from facility-approved vehicles. Ensures safe transportation of residents to scheduled medical appointments or other approved activities and outings. Maintains interior and exterior of facility vehicles to promote safe and comfortable transport and project a positive image in the community. Monitors vehicle maintenance logs and delivers vehicle for regularly scheduled maintenance such as oil changes, tire rotations, etc. Performs all other duties as assigned.

Responsibilities

Must be flexible with work time (in case a resident appointment or activity runs late). Must have training and be able to demonstrate safe clinical-based transfer techniques for:

• Ingress/Egress of Mobile Patients with Van
• Transfer of patients into and out of Wheelchair
• Properly securing WC-bound residents into bays in Vans

Must be able to lift transfer and mobilize patient weights of up to 70 pounds by self in confined areas inside Van. Must be able to make complex decisions involving highly dependent patient base.

Supervisory Requirements

This position has no supervisor responsibilities.

Qualifications – Education and/or Experience

• Minimum of a High School Diploma or a GED.
• Minimum age 25 years old.
• Must have a valid driver’s license.
• Must be familiar with GPS and local driving area.
• No traffic crimes in the last 7 years. DUI, DWS, Reckless driving, no traffic felonies. Multiple DUI or DWS, leaving scene with injuries, assault with vehicle. No more than 2 moving violations in 3 years. No more than one accident in 3 years.
• Pass a drug screen and background check.

Language Skills

Ability to read, write, and understand job-related instructions and manuals. Ability to effectively present information and respond to questions from managers, coworkers, and residents.

Mathematical Skills

Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability

Ability to solve practical problems. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Certificates, Licenses, and Registrations

Must be proficient in Microsoft Office Suite. Must have successfully completed CNA training and maintain all continuing education to maintain certification.

Physical Demands
