Job summary

HarmonyCares is seeking physicians to deliver in-home primary care for high-acuity geriatric patients with complex medical needs. You will provide comprehensive patient care management in the home and coordinate with an integrated care team to improve outcomes.

Qualified candidates hold an active/unrestricted medical license and DEA, board certification or eligibility, and EMR experience. A valid driver’s license and clean driving record are required to visit patients across the service area.

Qualifications

• Experience in geriatrics or complex patients preferred.
• Must maintain a valid driver's license and good driving record.

Responsibilities

• Provide primary health care to homebound patients.

• Collaborate with multidisciplinary team to optimize care plans.
