Zohorecruit is seeking a Primary Care Physician to join a senior-focused, value-based care practice in New Port Richey, FL. The role emphasizes longer visits, small panels, and robust support, with opportunities for partnership after 2 years.

Qualifications

• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics.
• ACN license accepted.

Responsibilities

• Provide comprehensive primary care to Medicare Advantage and senior patients 65+.
• Perform thorough assessments and develop individualized care plans.
• Manage chronic conditions, preventive care, and acute medical concerns.
• Deliver patient-centered care within a managed care/value-based model.
• Collaborate with physicians, specialists, and interdisciplinary teams to coordinate plans.
• Identify and close care gaps while supporting quality initiatives.
• Educate patients and caregivers on prevention and healthy lifestyle.
• Build strong relationships through compassionate, personalized care.
