Job summary

Palm Medical Centers is seeking a Primary Care Nurse Practitioner to join our value-based primary care team in Florida. You will provide comprehensive care to Medicare Advantage/Medicaid populations, emphasizing prevention and chronic disease management in a patient-centered setting.

We offer a strong compensation package with benefits, a 8am-5pm schedule, Monday to Friday, no on-call, and opportunities for professional growth within a large, stable organization.

Qualifications

• 2-3 years experience in primary care/value-based care setting (Medicare Advantage/Medicaid)
• Active Florida APRN license
• DEA license or eligible within 2 months
• Strong understanding of chronic disease management in value-based care
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus)
