Job summary

Palm Medical Centers is seeking a Primary Care Nurse Practitioner to join our premier team in Florida. You will provide patient-centered, value-based care for Medicare Advantage, Medicare and Medicaid populations in a supportive in-person environment with autonomy and excellent work-life balance.

We offer a strong compensation package with competitive benefits, an 8am–5pm Monday–Friday schedule with no on-call, and the resources to make a major impact on patients’ quality of life.

Qualifications

• 2–3 years of experience in primary care/value-based care setting
• Active Florida APRN license
• DEA license or eligible within 2 months of hire
• Strong understanding of chronic disease management in value-based care
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus)

Responsibilities

• Provide primary care services to Medicare/Medicaid populations
• Coordinate care with interdisciplinary team
• Manage chronic diseases and preventive care
• Maintain comprehensive and accurate patient documentation
• Develop and sustain patient relationships to ensure quality outcomes
